After much experimentation, our best solution turned out to be "naked" cable (i.e. Internet only), MagicJack (it's great), and DirecTV (superior HDTV as compared to cable, and no, phone line is not required... order PPV via directv.com). For backup, we also have two cell phones with different carriers.
